कोरोना पर विजय पाने के लिए बार-बार टीका लगवाना होगा

भारत में कोरोना वायरस के संक्रमण की स्थिति यूरोप के कई देशों की अपेक्षा इस समय कहीं बेहतर है। लेकिन, पिछले दो वर्षों का अनुभव दिखाता है कि यह स्थित बहुत देर तक नहीं रहती। यूरोप या अमेरिका में आज जो कुछ देखने में आ रहा है, कुछ ही दिनों में वह भारत की भी व्यथा-कथा बन सकता है। यूरोप में इस समय सबसे गंभीर स्थिति सवा आठ करोड़ की जनसंख्या वाले सबसे बड़े और सबसे साधन संपन्न देश जर्मनी की है।

जर्मनी में इस समय हर दिन ढाई से तीन लाख नए संक्रमण दर्ज हो रहे हैं। 24 मार्च के दिन यह संख्या पिछले दो वर्षों के सारे रिकॉर्ड तोड़ते हुए 3,18,387 हो गई। एक ही सप्ताह में 15 लाख लोग बीमार पड़ गए। कल-कारख़ाने और कार्यालय ही नहीं, अस्पतालों, स्वास्थ्य सेवाओं, पुलिस विभाग, सड़क और रेल परिवहन सेवाओं, यहां तक की आग बुझाने वाली दमकल सेवाओं के भी इतने सारे लोग या तो स्वयं संक्रमित हैं या परिवार में कोई संक्रमित है कि उन्हें पृथकतावास के नियमों का पालन करना पड़ रहा है।

कोरोना के नए संस्करण ओमीक्रॉन और डेल्टा बीए.2 का यह प्रकोप जर्मनी में हर दिन ढाई से तीन सौ लोगों के प्राण भी ले रहा है। 'सिर मुंड़ाते ही ओले पड़े' वाली यह मुसीबत तब आ पड़ी है, जब 20 मार्च से जर्मनी में कोरोना संबंधी रोक-टोक के बहुत सारे नियम हटा लिए गए हैं या उनमें ढील दी गई है। यूरोप के अन्य देशों में स्थिति जर्मनी की अपेक्षा बेहतर है, पर कोरोना से पूरी तरह मुक्त कोई नहीं हैं। 

जर्मनी की प्रोफ़ेसर मैरीलीन अद्दो, हैम्बर्ग के विश्वविद्यालय अस्पताल में कोविड-19 जैसी नई संक्रामक बीमारियों से लड़ने के उपायों पर शोधकार्य कर रही हैं। उनकी रोकथाम के लिए वे स्वयं भी एक टीका विकसित करना चाहती हैं। कोरोना की नई लहर के बारे में उनका कहना है कि बहुत से लोग उसे गंभीरता से लेने के बदले यह सोचते हैं कि वह बहुत प्राणलेवा तो है नहीं, इसलिए उसका संक्रमण लगने से उनके शरीर की रोगप्रतिरक्षण-शक्ति (इम्यूनिटी) अपने आप बढ़ेगी। लेकिन, यह एक भ्रम है। संक्रमण से मिली प्रतिरक्षण-शक्ति और टीके से मिली प्रतिरक्षण-शक्ति में बहुत अंतर है।

इस अंतर का उदाहरण देते हुए प्रो. अद्दो कहती हैं, ''खसरा (मीज़ल) और गलसुआ (मम्प्स) हो जाने पर अतीत में अक्सर कहा जाता था कि इस तरह से मिली प्राकृतिक-प्रतिरक्षण शक्ति सबसे अच्छी होती है। लेकिन, फ्लू या कोरोना वायरस जैसे वायरसों के मामले में– जो सांस की बीमारियों के वायरस हैं– यह नियम अधिकतर काम नहीं करता। श्वसन रोगों वाले वायरसों से पैदा हुई प्रतिरक्षण-शक्ति बहुत देर तक नहीं टिकती। सार्स-कोव-2 के संक्रमण से भी कुछ प्रतिरक्षण-शक्ति मिलती ज़रूर है, पर समय के साथ वह घटती जाती है। प्राकृतिक प्रतिरक्षण-शक्ति भी श्वसन रोगों वाले वायरसों से हमेशा लड़ नहीं पाती। इसी प्रकार, ठीक हो जाने के बाद किसी के रक्त में अधिक एन्टीबॉडी (प्रतिकारक) होते हैं तो किसी के रक्त में कम। टीका लगवाने का लाभ यह है कि हमारे रोगप्रतिरक्षण तंत्र में एक समरूपता-सी आ जाती है, हालांकि तब भी कुछ कमियां रह सकती हैं।''

जर्मनी में 5000 वयस्कों के बीच किए गए एक अध्ययन में पाया गया कि जिन लोगों को दो बार कोरोना से बचाव का टीका लग चुका है, उनके संक्रमित होने की संभावना कम से कम छह महीनों तक के लिए 77 प्रतिशत तक घट जाती है। समस्या यह है कि जर्मनी में कोरोना से बचाव के सभी टीके सबके लिए पूरी तरह मुफ्त होते हुए भी उनका का विरोध इतना प्रबल है कि अब तक केवल 76 प्रतिशत लोगों ने पहला टीका लगवाया है और 58.4 प्रतिशत ने दूसरा टीका भी। 23.5 प्रतिशत ने कोई टीका नहीं लगवाया है। अधिकतर वे ही और बहुत वयोवृद्ध लोग बार-बार संक्रमित होते हैं। जर्मनी के उत्तरी पड़ोसी देश डेनमार्क में 84 प्रतिशत लोगों ने कम से कम एक टीका लगवा लिया है, इसलिए वहां सारे रोक-टोक फ़रवरी के अंत में ही उठा लिए गए।  

प्रो. अद्दो नहीं मानतीं कि जर्मनी में या कहीं भी, जिन लोगों ने टीका नहीं लगवाया है, उनमें टीका नहीं लगा होने की कमी ओमीक्रॉन के संक्रमण से पूरी हो सकती है। कहती हैं, ''इसे साफ़-साफ़ समझ लिया जाना चाहिए कि कोई टीका नहीं लगा होने की कमी ओमीक्रॉन का संक्रमण कतई पूरी नहीं कर सकता। यही नहीं, मूलभूत टीके और बूस्टर टीका लगवाना भी सबका लक्ष्य होना चाहिए। गर्मियों के दिन तो शायद ठीक-ठाक बीत जाएंगे, पर सर्दियां चुनौती बन सकती हैं। जितने अधिक लोग सभी टीके लगवा चुके होंगे या बीमारी से उबर चुके होंगे, सर्दियों में उतनी ही कम चुनौतियां पैदा होंगी, हालांकि हमें अभी से यह मानकर भी चलना होगा कि सर्दियां आने के साथ एक नई संक्रमण लहर भी आएगी। अब तक का अनुभव यही दिखता है कि टीका न लगा हो और आयु 60 साल से अधिक हो, तो बीमारी के गंभीर बन जाने की संभवना बहुत बढ़ जाती है।''

प्रो. अद्दो का यह भी मानना है कि ''समय के साथ न केवल टीके का प्रभाव घटता जाता है, नए-ऩए वायरस या उनके नए-नए संस्करण भी बनते रहते हैं। अतः भविष्य में भी हमें नियमित रूप से उसी तरह नवीकरणीय एवं बूस्टर टीके लगवाते रहना होगा, जिस तरह फ्लू से बचाव के लिए हर साल नया टीका लगवाना पड़ता है। कम से कम उन लोगों को तो हर साल ऐसा करना ही पड़ेगा, जो नाज़ुक स्वास्थ्य वाले हैं।''  

इसराइल में देखा गया था कि 60 साल से अधिक आयु वालों को चौथा टीका लगाने से उन्हें संक्रमित होने से बचाने में, या संक्रमित हो जाने पर उनकी बीमारी की गंभीरता को नियंत्रित रखने में काफ़ी सहायता मिलती है। इसे देखने के बाद जर्मनी तथा कई दूसरे देशों नें भी 60 साल से अधिक आयु वालों को चैथा टीका लगवाना भी शुरू किया। फ़िलहाल यह नहीं कहा जा रहा है कि सभी लोग चौथा टीका लगवाएं। लेकिन जिन लोगों ने अभी तक कोई टीका नहीं लगवाया है, किंतु अब लगवाना चाहते हैं, उन्हें सलाह दी जा रही है कि वे कोविड-19 के लिए प्रमाणित कोई टीका लगवाएं, न कि ओमीक्रॉन पर लक्षित कोई टीका चुनें। ऐसा इसलिए, क्योंकि ओमीक्रॉन पर लक्षित विशेष टीकों की प्रभाकारिता के बारे में तथ्य और आंकड़े अभी उपलब्ध नहीं हैं।

दुनिया की विभिन्न प्रयोगशालाओं में कोविड-19 के नए-नए टीके बनाने के प्रयास अब भी चल रहे हैं। उनकी उपयोगिता के बारे में प्रो. मैरीलीन अद्दो का मत है कि हमें सैकड़ों अलग-अलग टीके तो नहीं चाहिए। लेकिन तथ्य यह भी है कि अफ्रीका सहित दुनिया के बहुत-से देशों में अब भी इन टीकों की भारी कमी है। इसी प्रकार यह भी देखने में आया है कि कोई टीका इस आयुवर्ग के लिए उपयुक्त नहीं है तो कोई किसी दूसरे आयुवर्ग के लिए उपयुक्त नहीं है।

उदाहरण के लिए, जर्मनी की टीका परामर्श समिति ने कहा कि 30 वर्ष से कम आयु के लोगों को केवल बायोनटेक/फ़ाइज़र का टीका लगाया जाए। यह उचित ही है कि वर्ग विशेष को ध्यान में रखते हुए उनके उपयुक्त अलग-अलग टीके हों। यह भी देखने में आया है कि अलग-अलग तकनीकों वाले टीकों के मेल से बेहतर परिणाम मिलते हैं। एक ही व्यक्ति को यदि एक बार वेक्टर-विधि से बना टीका लगाया गया है तो दूसरी बार उसे मेसेंजर-आरएनए (mRNA) विधि से बना टीका लगाने की सलाह इसी कारण दी जाती है। 

दोनों विधियां शरीर की कोशिकाओं को बताती हैं कि उन्हें वे 'स्पाइक प्रोटीन' कैसे बनाने हैं, जो कोरोना वायरस की बाहरी सतह पर होता है। यह 'स्पाइक प्रोटीन' बनते ही शरीर का रोगप्रतिरक्षण तंत्र जान जाता है कि यह चीज़ तो अपने शरीर का हिस्सा नहीं है। इसलिए प्रतिरक्षण तंत्र उसे बाहरी मान कर तुरंत वे एन्टीबॉडी बनाना शुरू कर देता है, जो बाद में शरीर में पहुंचे उसी जैसे स्पाइक प्रोटीन वाले कोरोना वायरसों को पहचान कर उन्हें नष्ट करने लगते हैं।

प्रो. मैरीलीन अद्दो कहती हैं कि अब तक के टीके वैसे परिणाम नहीं दे पाए, जैसी अपेक्षा थी, इसलिए नए टीके बनाने काम भी चलते रहना चाहिये। वे स्वयं भी कोरोना से लड़ने वाला एक नया टीका बनाने का प्रयास कर रही हैं। उनके सामने समस्या यह आ रही है कि अधिकतर लोगों को पहले ही टीका लग जाने से उन्हें ऐसे लोग बहुत ही कम मिल रहे हैं, जिन पर वे अपना टीका आजमा सकें। इसलिए वे कोई मौलिक टीका बनाने के बदले वेक्टर विधि वाले एक बूस्टर (प्रवर्धक) टीके के साथ प्रयोग कर रही हैं। उन्होंने पूरी तरह अहानिकारक चेचक वाले टीके की सामग्री को अपने बूस्टर का आधार बनाया है। चेचक का वायरस एक वेक्टर के तौर पर लंबे समय से इस्तेमाल होता रहा है। उसकी सहायता से रोगप्रतिरक्षण तंत्र को सार्स-कोव-2 वायरस के स्पाइक प्रोटीन का आभास दिया जाता है। 

इस बीच इंजेक्शन की सुई द्वारा टीका लगाने के बदले ऐसे टीके बनाने पर भी काम हो रहा है, जो नाक द्वरा दिये या खींचे जा सकते हैं। आशा की जा रही है कि इन टीकों की सामग्री नाक की श्लेष्मा झिल्ली में रह कर वायरसों को वहीं रोक कर नष्ट कर देंगी, ताकि वे श्वासनली या फेफड़ों तक पहुंच ही न पाएं। प्रयोगशालाओं में इस तरह के पहले अध्ययन शुरू हो गए हैं। प्रो. अद्दो ने बताया कि टीके बनाने वाली कुछ कंपनियां और प्रयोगशालाएं कई स्पाइक प्रोटिनों वाले टीके बनाने में भी लगी हुई हैं। एक ही टीके में कई प्रकार के स्पाइक प्रोटिन होने से कई प्रकार के वायरसों या एक ही वायरस के अलग-अलग संस्करणों से एकसाथ निपटना संभव हो जाएगा। प्रकृति और मनुष्य के बीच 'तू डाल-डाल, तो मैं पात-पात' वाली यह लड़ाई अभी लंबी चलेगी। (लेखक वरिष्ठ पत्रकार हैं एवं एक दशक तक डॉयचे वेले की हिन्दी सेवा के प्रमुख रह चुके हैं।)
