भारत और बांग्लादेश में डायबिटीज की बीमारी का आम होना कहीं ना कहीं सेहत और खानपान से जुड़ी समस्या की ओर इशारा करता है। यह टॉप 10 जानलेवा बीमारियों में से एक है।
दिल्ली में रहने वाला 17 साल का रोहिन सरीन अपनी डायबिटीज के बारे में कहता है, "यह कभी ठीक नहीं होने वाला।" उसके स्कूल के दोस्तों को भी पता है कि उसे हमेशा अपने साथ कोई खास चीज लेकर चलना पड़ता है। कभी सिर चकराने या तबीयत खराब सी लगने पर उन्होंने रोहिन को स्कूल बैग से उसका इंसुलिन पेन निकाल कर इंजेक्शन लेते देखा है। वह रोज ऐसे चार इंजेक्शन लेता है और कुछ खाता है ताकि खून में ब्लड शुगर की मात्रा सुधर जाए।
भारत में ऐसा जीवन जी रहे लोगों की कोई कमी नहीं है। अंतरराष्ट्रीय डायबिटीज फेडरेशन का अनुमान है कि 8.8 प्रतिशत भारतीयों को मधुमेह है। प्रतिशत में कम लगने वाले इस आंकड़े का भारत जैसी बड़ी आबादी वाले देश में मतलब होता है करीब 11.5 करोड़ लोग, जिनमें से कइयों को तो पता भी नहीं है कि उन्हें डायबिटीज है।
पश्चिम बंगाल के दक्षिण 24-परगना में रहने वाली 16 साल की रुकसाना को तीन साल से डाइबिटीज है। इस बीमारी के कारण उसका खाना-पीना और जीवनशैली रातोंरात बदल गई। अब वह घर में बिना तेल-मसालों वाली सब्जियों के साथ रोटी खाती है। उसके साथ गांव के तालाब से पकड़ी गई मछलियां भी खा लेती है। वह कहती है, "शुरू में मुझे अपने पसंदीदा चीजों को छोड़ने का दुख हुआ था। लेकिन जीवन मैं बहुत कुछ छोड़ना पड़ता है। अगर लंबे समय तक जीना है तो जीवन में संयम तो बरतना ही होगा।”
रुकसाना ने इसी साल 10वीं की परीक्षा पास की है। अब गांव में हायर सेकेंड्री स्कूल नहीं होने की वजह से उसे गांव की दूसरी लड़कियों के साथ आगे की पढ़ाई के लिए साइकिल से पांच किमी दूर नजदीकी स्कूल जाना होगा।
चीन के बाद सबसे ज्यादा डायबिटीज प्रभावित वयस्क लोग भारत में ही रहते हैं। इंडियन काउंसिल ऑफ मेडिकल रिसर्च (ICMR) की इंडिया डायबिटीज स्टडी नेम बताया गया है, "डायबिटीज और अन्य गैर-संक्रामक बीमारियों जैसे डिलिपिडीमिया, हाइपरटेंशन, मोटापा और मेटाबोटिक सिंड्रोम सार्वजनिक स्वास्थ्य पर बहुत बड़ा और निरंतर बढ़ने वाला बोझ बनते जा जा रहे हैं।”
आखिर इसका कारण क्या है? हुआ यह कि आर्थिक वृद्धि ने औसत भारतीयों की जीवनशैली बदल दी। लोग बाहर का खाना खूब खाने लगे और घर के पारंपरिक सादे खाने के बजाय फास्ट फूड पर जोर देने लगे। इन बदलावों के कारण समाज में मोटापा बहुत तेजी से फैलाने लगा।
तुरत फुरत वाला खाना
ऐसा नहीं कि समस्या केवल शहरों के अमीर लोगों तक सीमित है। एक जर्मन संस्था से सहयोग करने वाली भारत की कैथोलिक हेल्थ एसोसिएशन ऑफ इंडिया (सीएचएआई) में डॉक्टर समीर वलसांगकर का कहना है, "आज भारतीय लोग पहले से कहीं ज्यादा प्रोसेस्ड उत्पाद खाने लगे हैं और पहले से कम हिलते डुलते हैं। इसके अलावा खाने में कार्बोहाइड्रेट की मात्रा भी ऊंची होती है।” वह कहते हैं कि इसी कारण लोग इस दुष्चक्र में फंस गए हैं।
खान पान की आदतों में मूलभूत बदलाव देखने को मिल रहे हैं। शहरों में लोग कम से कम सब्जियां, फल और अनाज खाने लगे हैं। डॉ. वलसांगकर कहते हैं, "एक किलो टमाटर 40 रुपए में मिलता है जबकि एक पैकिट मैगी नूडल्स आपको 10 रुपए में मिल जाते हैं। इस तरह गरीब जनता के लिए भी यह आकर्षक विकल्प बन जाता है। कई बच्चे तो दिन में दो तीन बार तक मैगी खाते हैं।”

चुपचाप जान लेती बीमारी
डायबिटीज कई तरह की होती है। ज्यादातर भारतीयों को टाइप 2 डायबिटीज है। यह बीमारी तब होती है जब किसी व्यक्ति के ज्यादा वजन के कारण उसके शरीर की इंसुनिल पैदा करने या इस्तेमाल करने की क्षमता कम हो जाती है। इंसुलिन वह एंजाइम है जो शरीर के भीतर खाने को तोड़ कर ऊर्जा में बदलता है। वहीं टाइप 1 डायबिटीज में तो शरीर में इंसुलिन बनता ही नहीं है।
भारत में एक और समस्या यह भी है कि लोगों को इसका पता ही नहीं होता। साथ ही उसका ख्याल न रखने के कारण डायबिटीज जानलेवा हो सकती है। इसीलिए भारत और पड़ोसी बांग्लादेश में जान लेने वाली बीमारियों की टॉप 10 सूची में डायबिटीज का भी नाम आता है।
डॉ. वलसांगकर कहते हैं कि ‘चुपचाप' रहने वाली इस बीमारी में थकान और प्यास जैसे लक्षण होते हैं जिनसे सीधे तौर पर किसी बीमारी का पता नहीं चलता। वह कहते हैं, "भारत को छोड़कर कई दूसरे देशों में 40 की उम्र से बाद हर व्यक्ति का नियमित डायबिटीज चेकअप होता है। हमारे हेल्थकेयर सिस्टम पर इतना बोझ है कि न तो टेस्ट करने के लिए पर्याप्त मशीनें और न ही इलाज करने के लिए पर्याप्त दवाएं हैं। कई मामलों में तो इसका पता लगने में इतनी देर हो जाती है कि लोग अंधे हो जाते हैं, कोई अंग खो देते हैं या उन्हें किडनी की कोई बीमारी हो जाती है।
बांग्लादेश में है सबसे बुरा हाल
भारत के पड़ोसी बांग्लादेश में हालात और बुरे हैं। एक करोड़ से ज्यादा बांग्लादेशी डायबिटीज से ग्रस्त हैं। सन 2045 तक इसमें और एक तिहाई की वृद्धि होने का अनुमान है। वैकल्पिक नोबेल पुरस्कार कहा जाने वाला "राइट लाइवलिहुड एवार्ड" जीत चुके जफरुल्ला चौधरी बांग्लादेश के एक डॉक्टर और एक्टिविस्ट हैं। वे इसका कारण खानपान की आदतें, व्यायाम की कमी और धूम्रपान बताते हैं।
डॉयचे वेले से बातचीत में उन्होंने बताया कि ग्रामीण इलाकों में मछली खाने को नहीं मिलती क्योंकि वे इसे बेचकर जीविका कमाते हैं, खाने की चीजों के दाम बढ़ गए हैं। वे कहते हैं कि इसी कारण "लोग कार्बोहाइड्रेट ज्यादा खाने लगे यानी ज्यादा चावल और कम सब्जी। साथ ही कोका कोला और कई तरह की एनर्जी ड्रिंक पीने लगे, जो महंगी होती हैं।”
77 साल के हो चुके चौधरी ने सन 1972 में बांग्लादेश में ग्रामीण स्वास्थ्य संगठन गॉनोशास्थ्या केंद्र स्थापित किया था। वे जोर देते हैं कि बीमारियों से बचाव के बारे में ज्यादा से ज्यादा जानकारी और स्वास्थ्य शिक्षा दिए जाने की जरूरत है।
रोहिन ने बदली आदतें
दिल्ली के युवा रोहिन ने भी डायबिटीज का पता लगने का बाद से अपने जीवन में कई चीजें बदली हैं। लगभग हर सुबह वह क्रिकेट के मैदान पर खेलकूद करने पहुंचता है। बीमारी के साथ जीते हुए पिछले सात सालों में उसने मीठा खाना और मीठी सॉफ्ट ड्रिंक से दूर रहना भी सीख लिया है।
