क्रिस्टीन रो, बीबीसी फ़्यूचर
'रात के वक़्त यहां बहुत मच्छर होते हैं। हम बाहर नहीं रह सकते हैं।' युवा माधवी शर्मा मुस्कुराते हुए ये बात हमें बताती हैं। उनका परिवार धान की खेती करता है और जानवर पाल कर अपना गुज़र-बसर करता है।
माधवी, भूटान के दक्षिणी इलाक़े में स्थित गांव समटेलिंग की रहने वाली हैं। माधवी का परिवार शाम का वक़्त किचेन में गुज़ारता है। वहां वो खुले चूल्हे में खाना पकाते हैं। इसके धुएं से कीड़े-मकोड़े भाग जाते हैं।
माधवी शर्मा ने दसवीं तक पढ़ाई की है। वो भी मुंहज़बानी और सरकारी अभियानों की मदद से उन्हें मलेरिया और उन्हें डेंगू के ख़तरों का अच्छे से अंदाज़ा है। दोनों ही बीमारियां मच्छरों से फैलती हैं।
माधवी का परिवार कभी भी अपनी रिहाइश के आस-पास पानी नहीं रुकने देता। वो मच्छरदानी लगाकर सोते हैं, जिन्हें सरकार की तरफ़ से बांटा गया है। माधवी के 14 महीने के बच्चे के लिए एक खटोला है। उस में भी मच्छरदानी लगी हुई है।
शर्मा परिवार के घर में साल में दो बार कीटनाशकों का छिड़काव होता है। परिवार ने घर की दीवारों पर मिट्टी और गोबर का लेप लगा रखा है। घर की दीवारों के बीच दरारें हैं। इस वजह से मच्छरदानी की ज़रूरत और बढ़ जाती है।
भूटान में मलेरिया कितनी बड़ी समस्या थी
मच्छरदानी, कीटनाशकों का छिड़काव और लोगों को मच्छरों के ख़तरे के प्रति जागरूक करना, सब भूटान सरकार के मलेरिया उन्मूलन कार्यक्रम का हिस्सा है। ये कार्यक्रम 1960 के दशक में शुरू हुआ था। लेकिन इसका असर 1990 के बाद दिखना शुरू हुआ।
1994 में भूटान में मलेरिया के 40 हज़ार से ज़्यादा मामले सामने आए थे। इन मरीज़ों में से 68 की मौत हो गई थी। लेकिन, 2018 के आते-आते मलेरिया के केवल 54 केस सामने आए।
इनमें से भी केवल 6 ही भूटान के मूल निवासी थे। मलेरिया से भूटान में 2017 में 21 साल के एक सैनिक की मौत हो गई थी। ये सैनिक भूटान की भारत से लगने वाली सीमा पर तैनात था। वो अस्पताल इतनी देर से पहुंचा था कि डॉक्टर उसे नहीं बचा सके।
मलेरिया की वजह से लोगों के काम करने की क्षमता घट जाती है। उनकी ख़ुशहाली कम हो जाती है। और बहुत बिगड़े मामलों में जान भी चली जाती है। इसीलिए, भूटान के अधिकारी इस बीमारी को अपने देश से जड़ से मिटाने की मुहिम चला रहे हैं। लेकिन, इस मिशन में क़ामयाबी के लिए भूटान को अपने विशाल पड़ोसी भारत से सहयोग की ज़रूरत होगी।
जब कोई देश मलेरिया से मुक्त होता है, तो ये जश्न की बात होती है। एक दशक में गिने-चुने देश ही ये क़ामयाबी हासिल कर पाते हैं। इससे देश के संसाधन दूसरी बीमारियों से लड़ने के काम आते हैं।
फिर, परजीवी कीटाणुओं का समूल नाश करने से उनके अंदर दवाओं से लड़ने की प्रतिरोधक क्षमता भी नहीं विकसित हो पाती है। क्योंकि मलेरिया से लड़ने के लिए गिनी-चुनी दवाएं ही उपलब्ध हैं।
मलेरिया से लड़ाई में क्या है चुनौतियां?
भूटान को ये देखने की भी ज़रूरत है कि क्या वो मलेरिया मुक्त देश बनने की मंज़िल तक पहले भी पहुंच सकता था? इस बीमारी से लड़ने की कोशिश कर रहे भूटान की एक धार्मिक चुनौती भी है। एक बौद्ध देश होने की वजह से भूटान में किसी भी जीव को मारना पाप माना जाता है।
भले ही वो बीमारी फैलाने वाला कीटाणु ही क्यों न हो। ऐसे में मलेरिया से बचने के लिए दवाएं छिड़कने वाले अधिकारियों को बहुत मुश्किलों का सामना करना पड़ता था।
भूटान के पहले कीट वैज्ञानिक रिनज़िन नामगे उन दिनों को याद कर के बताते हैं, 'हम लोगों को ये समझाते थे कि हम तो बस दवा छिड़क रहे हैं। अब कोई मच्छर यहां आकर ख़ुदकुशी करना चाहता है, तो उस में कोई क्या कर सकता है।'
आज से कई दशक पहले छिड़काव करने वालों को घरों के भीतर कई बार तो ज़बरदस्ती घुसना पड़ता था। क्योंकि लोग विरोध करते थे। जब हमारी मुलाक़ात ड्रक डायग्नोस्टिक सेंटर की ऑफ़िस मैनेजर डेनचेन वांगमो से हुई, तो बड़ा दिलचस्प वाक़या हुआ। जब हमने पूछा कि क्या हम किसी तिलचट्टे को पैर से दबा सकते हैं। तो डेनचेन ने कहा कि क़तई नहीं।
उन्होंने कहा, 'ऐसा करना पाप है। हालांकि वो बाद में हँस पड़ीं। डेनचेन, भारत से रोज़ भूटान मज़दूरी करने आने वालों की पड़ताल करती हैं। जांच ये होती है कि कहीं वो अपने साथ मलेरिया के परजीवी तो नहीं ला रहे हैं।'
मलेरिया पीड़ित को अस्पताल में रहना पड़ता है
अगर किसी मरीज़ को सिफलिस नाम की बीमारी का शिकार पाया जाता है, तो उसे वापस कर दिया जाता है। अगर कोई भारतीय कामगार मलेरिया से पीड़ित होता है, तो उसे तीन दिन तक अस्पताल में रहना होता है, ताकि उसका सही इलाज हो सके।
बहुत से भारतीय मज़दूर, मलेरिया के कीटाणु पाए जाने पर भूटान की सीमा से लौट आते हैं क्योंकि तीन दिन अस्पताल में रहने का मतलब उनके लिए तीन दिन की मज़दूरी गँवाना होता है।
भूटान से लगे हुए असम में जातीय हिंसा की वजह से बड़ी तादाद में लोग भाग कर भूटान में पनाह लेते थे। इसकी वजह से मलेरिया उन्मूलन का अभियान बहुत प्रभावित हुआ था। कई बार इलाज करने वाले डॉक्टरों को पीटा गया और कुछ को तो जान से मारने की धमकी भी दी गई।
2016 तक भूटानी नागरिकों को वसूली के लिए अगवा भी किया जाता था। फिर भूटान और भारत के अधिकारियों के बीच आपसी सहयोग भी नहीं होता। भारत के लोगों को लगता है कि भूटान में पनबिजली की जो परियोजनाएं बन रही हैं, उनसे भारतीय इलाक़ों में बाढ़ आया करेगी।
भूटान और भारत का रिश्ता बहुत संवेदनशील भी है और पेचीदा भी। भूटान में जो पनबिजली परियोजनाएं लग रही हैं, उनसे बनने वाली बिजली का फ़ायदा ज़्यादातर तो भारत को ही मिलेगा।
हालांकि अब दोनों देशों के बीच सहयोग बढ़ रहा है। भूटान के मलेरिया और परजीवी रोग विशेषज्ञ टोबग्याल कहते हैं कि उनके कुछ मरीज़ तो उनके हाथों में मरे थे। अब वैसा नहीं होता।
मलेरिया उन्मूलन में भूटान ने जितनी तरक़्क़ी की है, उतनी भारत में नहीं हुई है। लेकिन, भूटान छोटा सा देश है। यहां ग़रीबी उन्मूलन में क़ामयाबी ने मलेरिया से निपटने में भी मदद की। यूं तो भूटान दुनिया के सबसे कम विकसित देशों में से है। लेकिन, जल्द ही वो भारत की तरह मध्यम आय वर्ग वाला देश बन जाएगा।
भूटान के गेलेफू रीजनल अस्पताल के मेडिकल सुपेरिंटेंडेंट दोरजी शेरिंग कहते हैं कि भूटान ने मलेरिया से जिस तरह मुक़ाबला किया है, वो कई देशों के लिए सबक़ है। सबसे बड़ा तजुर्बा तो ये है कि बीमारी के कीटाणु ले जाने वाले जीवों का नियंत्रण। जैसे कि मलेरिया के मच्छर।
भूटान को इस काम में ग्लोबल फंड टू फाइट एड्स, टीबी ऐंड मलेरिया से भी आर्थिक मदद मिली थी। हालांकि जब उसे मलेरिया मुक्त घोषित कर दिया जाएगा, तब ये मदद नहीं मिलेगी।
भूटान की क़ामयाबी की एक वजह सियासी प्रतिबद्धता भी थी। सरकार ने मज़बूत इरादे के साथ अभियान चलाया। फिर सामाजिक संगठनों को भी इससे जोड़ा गया। दोरजी शेरिंग कहते हैं कि हर एक इंसान ने अपने रोल की अहमियत और ज़िम्मेदारी समझी।

क़ामयाबी की वजह यहां के संस्कार
भूटान के मुक़ाबले भारत में मलेरिया के बहुत से मामले हर साल सामने आते हैं। 2018 में भारत का हर 265वां इंसान मलेरिया का शिकार हुआ। हालांकि इससे पहले के साल से ये आधा ही था।
भूटान की कामयाबी की वजह यहां के संस्कार हैं। यहां के नागरिक आमतौर पर क़ानून का पालन करने वाले हैं। सब को देश की तरक़्क़ी में योगदान देने का हक़ भी है और लोग ज़िम्मेदारी भी समझते हैं।
इसके मुक़ाबले भारत में ऐसा माहौल नहीं है। दुनिया का सबसे बड़ा लोकतांत्रिक देश भारत घर-घर में मलेरिया की दवा का छिड़काव करने के मिशन में भी सफल नहीं हो सका है। भूटान में सब के लिए इलाज की मुफ़्त व्यवस्था है। फिर चाहे वो भूटान के नागरिक हों या किसी और देश के।
मलेरियाः भूटान के मुक़ाबले भारत कहां?
वहीं भारत में ऐसा नहीं है। असम के टुकराझार के रहने वाले प्रमोद नरज़री को पिछली बार जब मलेरिया हुआ था, तो वो बच्चे थे।
वो कहते हैं कि भारत के सरकारी अस्पतालों की हालत ख़स्ता है। प्रमोद ख़ुद को ख़ुशनसीब मानते हैं कि उनके गांव के पास ही एक अस्पताल है और गांव मे एक क्लिनिक भी है। ऐसा नहीं होता तो वो किसी सरकारी अस्पताल में जाने के बजाय निजी अस्पताल में ही जाना पसंद करते। भूटान से लगी सी सीमा पर रहने वाले कई भारतीय तो इलाज के लिए भूटान जाते हैं।
भूटान में अब वहीं मलेरिया के मामले सामने आ रहे हैं, जो भारतीय सीमा से लगे हैं। सीमा के आर-पार बीमारी के विस्तार का मामला सिर्फ़ मलेरिया तक सीमित नहीं है। अफ्रीका के कॉन्गो और युगांडा के बीच इबोला वायरस की आमद-ओ-रफ़्त भी इसकी एक मिसाल है।
सीमा पर मलेरिया की बीमारी की वजह से कई देश इस पर पूरी तरह से काबू नहीं पा सके हैं। जैसे कि सऊदी अरब और यमन। हैती और डोमिनिकन रिपब्लिक। चीन और म्यांमार।
मलेरिया का जड़ से ख़ात्मा करने के लिए ज़रूरी है कि इस मामले में रियायत न बरती जाए। भूटान के साथ दिक़्क़त ये है कि वो मज़दूरों और दूसरे संसाधनों के लिए भारत पर निर्भर है।
तो, वो सीमा पार से आवाजाही पूरी तरह से बंद नहीं कर सकता। इसलिए सरहद पर तैनात बीमारी से लड़ने वाले प्रहरियों की सजगता ही इस बीमारी के ख़ात्मे का पहला मोर्चा हैं।
यूं तो भारत और भूटान, दोनों ही मलेरिया से मुक़ाबला कर रहे हैं। और इसके ख़ात्मे के क़रीब हैं। लेकिन, अगर दोनों आपसी सहयोग से काम लेते, तो ये सफ़र और जल्दी तय हो जाता।
