भारत की राजधानी दिल्ली में प्रत्येक चार में से एक शख़्स कोविड-19 से संक्रमित हो सकते हैं। यह आकलन नेशनल सेंटर फॉर डिजीज कंट्रोल की ओर से दिल्ली में कराए गए सर्वे का है।
इस सरकारी सर्वे के लिए दिल्ली में रैंडम ढंग से 21,387 लोगों के ब्लड सैंपल लिए गए। इनमें 23.48 प्रतिशत लोगों के ब्लड में कोविड-19 एंटीबॉडी पाया गया।
इस सर्वे के नतीजे से ज़ाहिर है कि कोरोना की स्थिति दिल्ली में जितनी बताई जा रही है उससे कहीं ज़्यादा ख़राब है।
दिल्ली में अब तक 1,23,747 लोगों के कोरोना संक्रमित होने की पुष्टि हुई है। यानी 1.98 करोड़ की आबादी वाले दिल्ली में एक प्रतिशत से भी कम लोगों में कोरोना की पुष्टि हुई है।
दिल्ली के ताज़ा सर्वे के मुताबिक से देखें तो 23.44 प्रतिशत के हिसाब से दिल्ली में 46.5 लाख लोगों में कोरोना संक्रमण होने की आशंका जताई जा सकती है।
लेकिन सरकार का कहना है कि अभी तक दिल्ली में ज़्यादा लोगों में कोरोना की पुष्टि नहीं होने की वजह यही है कि बड़ी संख्या में संक्रमित लोगों में कोरोना के कोई लक्षण नहीं देखे गए हैं। हालांकि यह भी आशंका जताई जा रही है कि दिल्ली में जिस तरह से आबादी का घनत्व है, उसमें यह आंकड़ा 23.44 प्रतिशत से भी ज़्यादा हो सकता है।
दिल्ली में एक साथ तीन टेस्ट : जून के अंत में देश के गृह मंत्री अमित शाह ने कहा था कि दिल्ली में कम्युनिटी ट्रांसमिशन नहीं हैं।
ऐेसे में सवाल पूछे जा रहे हैं कि तो फिर देश की राजधानी दिल्ली में क्यों कराए जा रहे हैं सीरोलॉजिकल टेस्ट? क्या सीरोलॉजिकल टेस्ट और कम्युनिटी ट्रांसमिशन का आपस में कोई लिंक है?
दरअसल, कम्युनिटी स्प्रेड का पता लगाने में वैज्ञानिक इस तरह के टेस्ट जिसे सेरो सर्वे भी कहा जाता है, उसे अहम मानते हैं।
दिल्ली में 25 जून को तय किया गया कि यहां तकरीबन 21 हजार लोगों का सीरोलॉजिकल टेस्ट कराया गया। 27 जून को इसकी शुरुआत हुई। पहले दिन तकरीबन 677 सैम्पल लिए गए।
10 जूलाई तक राजधानी दिल्ली में इस टेस्ट को पूरा करने की बात कही जा रही है, लेकिन नतीजा आते-आते 21 जुलाई का वक़्त लग गया।
कैसे किए जाते हैं सीरोलॉजिकल टेस्ट? : आमतौर पर कोविड-19 के लिए टेस्ट में नाक और गले से स्वैब निकाला जाता और फिर RT-PCR टेस्ट किया जाता है। इसके नतीजे़ आने में 6-8 घंटे का वक़्त लगता है। इससे यह पता चलता है कि इंसान को टेस्ट के दौरान कोविड-19 बीमारी है या नहीं।
दिल्ली में इस वक्त कई तरह के कोरोना टेस्ट एकसाथ किए जा रहे हैं, उनमें से एक तो RT-PCR टेस्ट है।
दूसरा टेस्ट है एंटीजन टेस्ट। जो दिल्ली में 19 जून से शुरू किया गया है। यह कई मायनों में RT-PCR टेस्ट की तरह होता है। इसमें शरीर के एंटीजन का पता लगाया जाता है। ये ब्लड टेस्ट के जरिए किया जाता है। इसमें नतीज़े आने में आधे घंटे का वक़्त लगता है। यानी एंटीजन टेस्ट में नतीजे RT-PCR के मुकाबले जल्दी आते हैं। 27 जून से राजधानी दिल्ली में तीसरे तरह का टेस्ट भी शुरू किया गया है जिसे सीरोलॉजिकल टेस्ट कहते हैं।
सीरोलॉजिकल टेस्ट दरअसल एक तरीके का ब्लड टेस्ट है जो व्यक्ति के खून में मौजूद एंटीबॉडीज की पहचान करता है।
दिल्ली के सर गंगाराम अस्पताल में मेडिसिन डिपार्टमेंट के हेड डॉ. एसपी बायोत्रा के मुताबिक़ ब्लड में से अगर रेड ब्लड सेल को निकाल दिया जाए, तो जो पीला पदार्थ बचता है उसे सीरम कहते हैं।
इस सीरम में मौजूद एंटीबॉडीज़ से अलग-अलग बीमारियों की पहचान के लिए अलग-अलग तरह का सेरोलॉजिक टेस्ट किया जाता है। बावजूद इसके सभी तरह के सीरोलॉजिकल टेस्ट में एक बात कॉमन होती है और वह ये है कि ये सभी इम्यून सिस्टम द्वारा बनाए गए प्रोटीन पर फोकस करते हैं। शरीर का यह इम्यून सिस्टम यानी रोग प्रतिरोधक क्षमता बाहरी तत्वों द्वारा शरीर पर किए जा रहे आक्रमण को रोककर आपको बीमार पड़ने से बचाता है।
एंटीजन और सीरोलॉजिकल एंटीबॉडी टेस्ट में फ़र्क : इस फ़र्क को समझने के पहले एंटीजन और एंटीबॉडी के फ़र्क को समझें। आसान शब्दों में कहें तो एंटीजन बीमारी का कारण है और एंटीबॉडी बीमारी के कारण से उत्पन्न होता है। पहला कॉज़ है और दूसरा इफेक्ट।
डॉक्टर बायोत्रा के मुताबिक दोनों ब्लड टेस्ट ही हैं, लेकिन जिनका एंटीजन टेस्ट पॉज़िटिव आता है उसका मतलब वो फिलहाल कोरोना से संक्रमित है। लेकिन कोविड-19 के सीरोलॉजिकल एंटीबॉडी टेस्ट में पॉज़िटिव पाए जाने का मतलब होता है कि पहले कभी वह व्याक्ति कोरोना से संक्रमित रहा होगा। अब ठीक हो गया होगा।
डॉक्टर बायोत्रा बताते हैं कि एक परिवार में अगर एक व्यक्ति कोरोना पॉज़िटिव पाया जाता है तो एंटीजन टेस्ट के जरिए ये पता लगाया जा सकता है कि परिवार के दूसरे सदस्य क्या फ़िलहाल कोरोना संक्रमित है। इसके नतीजे एंटीजन टेस्ट से तुरंत सामने आ सकते हैं। इस तरह से उन्हें आइसोलेट और क्वारंटीन करने में मदद मिलेगी।
लेकिन सीरोलॉजिकल एंटीबॉडी टेस्ट की मदद से यह पता लगा सकते हैं कि क्या परिवार के दूसरे किसी सदस्य को पहले कभी कोरोना हुआ था? हो सकता है कि परिवार में किसी को हुआ हो और वो ठीक भी हो गया हो और उसमें लक्षण न पाए गए हों, लेकिन उसने परिवार के दूसरे सदस्य में कोरोना संक्रमण फैला दिया हो।
फिलहाल दिल्ली में ये टेस्ट नेशनल सेंटर फॉर डिजीज कंट्रोल और दिल्ली सरकार मिलकर रहे हैं। इस सर्वे के लिए सरकार ने आंगनवाड़ी, आशा वर्कर टीचर और नगर निगम के कर्मचारियों की भी मदद ली है, लेकिन प्राइवेट लैब्स को इजाजत नहीं मिली है।
डॉ. लाल पैथ लैब्स के हेड अरविंद लाल के मुताबिक़ उन्होंने आईसीएमआर से इसके लिए इजाजत मांगी है। उन्हें फ़िलहाल राज्य सरकारों से संपर्क करने को कहा गया है। यानी राज्य सरकारों से मंज़ूरी मिलने के बाद ही प्राइवेट लैब्स इस तरह के सर्वे में हिस्सा ले पाएंगे।
सीरो टेस्ट से हासिल क्या होगा? : यह पता लगाने के लिए बीबीसी ने बात की आईसीएमआर में वैज्ञानिक डॉक्टर मनोज मुरहेकर से।
डॉ. मनोज का मानना है कि इस तरह का टेस्ट यह पता लगाने के लिए किया जाता है कि इलाके के कितने प्रतिशत लोग इस बीमारी को एक्सपोज़ हो चुके हैं - वर्तमान में या फिर पहले भी।
देश में तकरीबन 70 ज़िलों में मई के महीने में 28 हजार लोगों के सीरोलॉजिकल टेस्ट किए गए थे। उस वक़्त दो फीसदी से भी कम लोगों में इस बीमारी के एंटीबॉडीज़ पाए गए थे। इसी आधार पर आईसीएमआर ने बताया था कि देश में कोरोना बीमारी कम्युनिटी ट्रांसमिशन के स्टेज में नहीं पहुंची है। आगे पूरे देश में ये टेस्ट दोबारा कब होंगे, इस बारे में डॉक्टर मनोज को कोई सूचना नहीं है।
डॉक्टर बायोत्रा के मुताबिक़ इस टेस्ट का इस्तेमाल किसी भी इमरजेंसी वाले पेशेंट में कोविड-19 का पता लगाने के लिए भी कर सकते हैं।
डॉक्टर बायोत्रा के मुताबिक़ इस वक़्त ऐसे टेस्ट की जरूरत दिल्ली को इसलिए है ताकि कंटेनमेंट प्लान में बदलाव की कोई गुंजाइश हो या फिर सरकार को अपनी टेस्टिंग की रणनीति में कोई बदलाव लाना हो तो तुंरत किया जा सके। कुल मिलाकर कोरोना के फैलाव को दिल्ली में रोकने के लिए ये टेस्ट जरूरी हैं।
हालांकि डॉक्टर मनोज कहते हैं कि फिलहाल देश में जो कंटेनमेंट की रणनीति चल रही है उसमें कोई बदलाव की जरूरत नहीं है। हां, सीरोलॉजिकल टेस्ट से ये संक्रमितों के ऐज प्रोफाइल का पता लगाया जा सकता है। दिल्ली के सर्वे में बच्चों को शामिल किया जा रहा है। बच्चों में कितने प्रतिश बच्चों में ऐसे एंटीबॉडी पाए गए हैं, ये पता लगा सकते हैं। इस सर्वे के नतीज़ों से कोरोना के भारत या फिर दिल्ली पर असर के तौर-तरीकों को स्टडी किया जा सकता है। नए कंटेनमेंट एरिया के चयन में इसका फायदा मिल सकता है।
केन्द्र का दावा है कि जहां 25 मार्च से 14 जून तक दिल्ली में केवल 2,41,000 टेस्ट हुए थे, वहीं केन्द्र सरकार के हस्तक्षेप के बाद 15 जून से 25 जून तक 1,75,141 टेस्ट हुए हैं। यानी 4.5 फीसदी का इज़ाफा हुआ है।
फिलहाल दिल्ली में इस वक़्त इस टेस्ट को कराने के पीछे ख़ास मकसद है। जून के महीने में अप्रत्याशित तरीके से कोरोना के मामले दिल्ली में बढ़े हैं।
इसके बाद दिल्ली के स्वास्थ्य मंत्री सत्येन्द्र जैन ने एक प्रेस कॉन्फ्रेंस में कह दिया था कि उन्हें लगता है कि दिल्ली में कम्युनिटी ट्रांसमिशन शुरू हो चुका है, लेकिन इस पर केन्द्र सरकार के कथन को ही अंतिम माना जाएगा।
