बीते रविवार को भारतीय प्रधानमंत्री नरेन्द्र मोदी ने कहा कि भारत में कोरोना वायरस संक्रमण को रोकने के लिहाज़ से अगले 3 से 4 सप्ताह बेहद अहम हैं।
भारत में कोरोना वायरस का पहला कंफ़र्म मामला 30 जनवरी को सामने आया था। इसके बाद इस महामारी को रोकने के लिए भारत ने कई क़दम उठाए हैं। भारत ने कोरोना वायरस संक्रमण को टेस्ट करने की अपनी क्षमता को बढ़ाया और महामारी रोकने के लिए लोगों की भीड़भाड़ को रोकने के लिए 122 साल पुराना सख्त क़ानून भी लागू कर दिया।
इसके अलावा अब 3 सप्ताह तक चलने वाले लॉकडाउन को 15 अप्रैल के बाद इस महीने के अंत तक बढ़ाने की तैयारी की जा रही है। भारत में 1 अरब से ज्यादा लोग अपने घरों में क़ैद हैं। सड़क, रेल और हवाई मार्ग से यातायात सेवाएं निलंबित हैं।
अब तक भारत में 1 लाख 80 हज़ार से ज्यादा लोगों का टेस्ट किया जा चुका है। इनमें 4.3 प्रतिशत सैंपल पॉज़िटिव पाए गए हैं। इस महामारी से अब तक भारत में 300 से ज़्यादा लोगों की मौत हो चुकी है। यह संक्रमण देश के 700 से ज्यादा ज़िलों में से आधे ज़िलों में फैल चुका है। इस संक्रमण के कई हॉटस्पॉट इलाक़ों को चिन्हित किया गया है।

भारत इस महामारी का सामना कैसे कर रहा है, इसको लेकर दुनियाभर के स्वास्थ्य विशेषज्ञों की नज़रें टिकी हैं। भारत का क्षेत्रफल भी बड़ा है, घनी आबादी है और बेहद कमज़ोर स्वास्थ्य सुविधाएं, ऐसे में संक्रमण फैलने का ख़तरा भी ज्यादा है। देश के एक प्रमुख वायरोलॉजिस्ट ने गोपनीयता की शर्त के साथ कहा कि इस डर से काफ़ी लोग चिंतित हैं। यह वायरस संक्रमण के शुरुआती दिन ही हैं और 3 से 4 सप्ताह में तस्वीर साफ़ होगी।
ब्रुकिंग्स इंस्टीट्यूशन के वरिष्ठ फैलो और अर्थशास्त्री शामिका रवि भारत में कोरोना संक्रमण के फैलने पर नज़र रख रही हैं। उनका कहना है कि भारत ने अब तक अच्छा किया है। उन्होंने बताया कि भारत में एक्टिव मामले 7 दिनों में दोगुने हो रहे हैं, यह पहले की तुलना में धीमी दर है। मौत की दर भी कम है लेकिन अब यह बढ़ रही है।
शामिका रवि ने कहा कि हमारे यहां संक्रमण की दर ज्यादा नहीं है और यह तब है, जब हमारे यहां आग से खेलने जैसी स्थित है। हमारे सारे टेस्ट पूरी तरह प्रोटोकाल से होते हैं। लोगों की ट्रैवल हिस्ट्री, फिर उनके संपर्क में आए लोगों की ट्रेसिंग और फिर जांच होती है।

हालांकि और पॉज़िटिव मामले मिलने की आशंका ज़्यादा है। कई लोग यह भी बता रहे हैं कि अस्पतालों में कोविड-19 संक्रमण और इंफ्लूएंजा जैसे बुख़ार को लेकर पहुंचने वाले लोगों की संख्या में तेज़ी आने की रिपोर्ट नहीं है, ऐसा होने पर यह कम्युनिटी ट्रांसमिशन का संकेत होता। लेकिन यह भी संभव है कि जानकारी की कमी और कमज़ोर रिपोर्टिंग के चलते ऐसा हो रहा हो।
इंदौर के एक निजी अस्पताल में मैंने मरीज़ों की संख्या में उछाल देखा था। इस अस्पताल में कोविड-19 के 140 से ज्यादा मरीज़ों का इलाज चल रहा था और इसमें क़रीब एक तिहाई मामलों में स्थिति गंभीर थी। रविवार तक अस्पताल में 1 दिन में 40 नए मामले पहुंच रहे थे। अस्पताल के चेस्ट स्पेशलिस्ट डॉ. रवि दोशी ने बताया कि हमें लगा था कि संक्रमण के मामले कम हुए है लेकिन अचानक से पिछले 2 दिनों में मरीज़ों की संख्या काफ़ी बढ़ गई है।
वहीं क्रिश्चियन मेडिकल कॉलेज वेल्लोर से वायरोलॉजी के रिटायर्ड प्रोफ़ेसर टी. जैकब जॉन जैसे एक्सपर्ट के मुताबिक़ भारत को बुरी स्थिति के लिए निश्चित तौर पर तैयार रहना चाहिए। टी. जैकब जॉन ने कहा कि हमें नहीं लगता है कि हम उस समस्या की गंभीरता को अब तक समझ पाए हैं, जो अगले 2 महीने में सामने आने वाली है। काफ़ी लंबे समय तक वायरस हमारी प्राथमिकताओं को निर्धारित करेगा।
डॉ. जॉन के मुताबिक़ भारत की अब तक की प्रतिक्रिया काफ़ी हद तक साक्ष्य आधारित रही है जबकि इसे अनुमान आधारित और कहीं ज्यादा सक्रिय होना चाहिए था। भारत का स्वास्थ्य मंत्रालय सख्ती से देश में कम्युनिटी ट्रांसमिशन से इंकार करता रहा है, हालांकि देशभर के कई डॉक्टरों के मुताबिक़ उन्हें मरीज़ों में कोविड-19 संक्रमण के लक्षण मार्च की शुरुआत से ही दिखने लगे थे।
डॉ. जॉन ने कहा कि पूरा फोकस इस बात पर है कि कम्युनिटी ट्रांसमिशन का साक्ष्य तलाशा जाए। यह रणनीतिक ग़लती है। हम सब जानते हैं कि कम्युनिटी ट्रांसमिशन की स्थिति है, वहीं डॉ. रवि के मुताबिक़ आने वाले दिनों का प्रत्येक सप्ताह बेहद अहम साबित होने वाला है।
आर्थिक सुस्ती पर रोक के लिए लॉकडाउन में ढील देने और कोरोना संक्रमण के ग्राफ को सपाट बनाने के लिए अब कहीं ज्यादा निगरानी की ज़रूरत होगी कि कौन संक्रमित है और कौन संक्रमित नहीं? इसके लिए भारत को लाखों टेस्टिंग किट और प्रशिक्षित तकनीशियनों की ज़रूरत होगी।
कोविड-19 संक्रमण का टेस्ट भी काफ़ी जटिल प्रक्रिया है। इसमें यह सुनिश्चित करना होता है कि दसियों हज़ार सैंपल लैब में ठीक से पहुंचाए जाएं। भारत के पास सीमित संसाधन हैं जिसकी क्षमता भी लिमिटेड है। ऐसे में डॉ. रवि कहती हैं कि 'पूल टेस्टिंग' का रास्ता ही बचा हुआ है।
विश्व स्वास्थ्य संगठन की अनुशंसाओं के मुताबिक़ पूल टेस्टिंग में नाक और गले के स्वैब के कई सैंपल ट्यूब में लिए जाएंगे और इन सबका एक ही बार में रियल टाइम कोरोना वायरस टेस्ट किया जाएगा। अगर टेस्ट निगेटिव हुआ तो इसका मतलब होगा कि सबकी रिपोर्ट निगेटिव है।
अगर रिपोर्ट पॉज़िटिव निकली तो फिर समूह के प्रत्येक व्यक्ति का अलग-अलग टेस्ट करना होगा। पूल टेस्टिंग से बड़ी आबादी का टेस्ट कम समय में संभव है। डॉ. रवि कहती हैं कि अगर कुछ ज़िलों में संक्रमण का कोई मामला नहीं है तो फिर उन ज़िलों को आर्थिक गतिविधियों के लिए खोला जा सकता है।
भारत के कई वायरोलॉजिस्टों का मानना है कि बड़े पैमाने पर लोगों का एंटीबॉडी टेस्ट- उंगली से रक्त सैंपल लेकर भी किया जाना चाहिए, इससे लोगों के शरीर में प्रोटेक्टिव एंटीबाडीज की मौजूदगी का पता चलेगा। माना जा रहा है कि पोलियो ड्रॉप देने की तुलना में ब्लड टेस्ट आसान भी है और तेज़ी से किया जा सकता है। भारत ने पोलियो ड्रॉप अभियान को सफलतापूर्वक पूरा किया है।
एक वायरोलॉजिस्ट ने बताया कि हमें एंटीबॉडी टेस्टिंग को डायग्नोस्टिक टूल के तौर पर नहीं बल्कि पब्लिक हेल्थ टूल के तौर पर इस्तेमाल करना चाहिए। हमें संक्रमण से रिकवर हुए लोगों की भी पहचान करने की ज़रूरत है ताकि उन्हें काम पर भेजा जा सके। इसके अलावा भारत को प्लाज्मा थैरेपी को भी देखने की ज़रूरत है।
एक वायरोलॉजिस्ट ने बताया कि इसमें संक्रमण से उबरने वाले मरीज़ की अनुमति से उनके रक्त की ज़रूरत होगी। इसमें ज्यादा एंटीबॉडी वाले रक्त प्लाज़मा को संक्रमित मरीज़ के रक्त में मिलाया जाता है। कई डॉक्टरों के मुताबिक़ इस संक्रमण के इलाज में यह उम्मीदभरा रास्ता हो सकता है।
ज्यादातर वायरोलॉजिस्टों का कहना है कि भारत में ज्यादा से ज्यादा लोगों को टेस्ट करने की ज़रूरत है। एक वायरोलॉजिस्ट ने बताया कि श्वसन संबंधी इंफेक्शन वाले हर मरीज़ का टेस्ट किया जाना चाहिए। भारत में संक्रामक रोगों के टेस्ट कराने की संस्कृति नहीं है, क्योंकि ज्यादातर नागरिक इसका ख़र्च नहीं उठा सकते। जोखिम को कम करना हमारी संस्कृति में नहीं है।
एक वायरोलॉजिस्ट ने बताया कि टेस्ट की जगह हमारा झुकाव इलाज की तरफ़ है। बीमारी के लक्षण और चिकित्सीय संकेत पर हम भरोसा करते हैं। संक्रमण के कारक और बीमारी की वजहों पर हम ध्यान नहीं देते। हम जब बीमार होते हैं तभी टेस्ट कराते हैं।
डॉ. जॉन के मुताबिक़ भारत सरकार इस वायरस के ख़िलाफ़ अपनी पूरी क्षमता से लड़ रही है लेकिन हो सकता है कि यह पर्याप्त नहीं हो।
कई लोगों की शिकायत है कि प्रधानमंत्री के उत्साह बढ़ाने के भाषण और ब्यूरोक्रेट्स की सूचनाओं वाली प्रेस ब्रीफिंग के अलावा इस महामारी से जुड़ीं जानकारियों में पारदर्शिता का अभाव है। महामारी किस रफ्तार से फैल रही है और जांच की दर क्या है, ये जानकारियां सहज उपलब्ध नहीं हैं। बीते सप्ताह ही मास्क पहनने को अनिवार्य बनाया गया है। लेकिन बेहतरीन सार्वजनिक स्वास्थ्य सुविधाओं और रिस्पांस के चलते केरल सरकार संक्रमण को काफी हद तक रोकने में कामयाब रही है।
एक वायरोलॉजिस्ट ने बताया कि यह लंबे समय तक चलेगा। संक्रमण रोकने के लिहाज़ से हम भारत को एक एपिसोड के तौर पर नहीं देख सकते। इस वायरस का असर इतनी जल्दी खत्म नहीं होगा। यह भी समझना होगा कि भारत के सभी राज्यों में संक्रमण के कम या ज्यादा मामले एकसाथ देखने को नहीं मिलेंगे। ऐसे में आने वाले सप्ताहों में ही पता चलेगा कि भारत में संक्रमण के मामले तेज़ी से बढ़ेंगे या भारत ने संक्रमण को फैलने से रोकने में कामयाबी हासिल कर ली है।
डॉ. जॉन बताते हैं कि यह ऐसी पहेली है जिसमें हज़ारों सवाल है। इनके जवाब देना आसान नहीं है।
